As duly elected president of the organization mentioned herein, I hereby make application for a franchise and the for the right to conduct a baseball and/or softball program under the name of Tar Heel Leagues, Inc. for this year.
By submitting this form, I pledge myself and my league to strict compliance of all the rules and regulations of the organization. I also agree to pay the franchise fees of $50.00 per team upon receipt of an invoice for the total amount.
